Order Requirements

ZooPharm is a pharmacy. All orders require a prescription from a licensed veterinarian, applicable DEA license and form(s).

DEA requirements for Class II controlled substances such as Carfentanil citrate and Etorphine HCl apply:

  • Purchaser must hold a current DEA license with appropriate drug class approvals
  • Placement on the Approved Carfentanil/Etorphine User's List
  • Submission of a completed DEA 222 form

Original copies of DEA Form 222 must be received for Class II controlled substances prior to the order being filled.
